Job Description

Responsible for servicing customer requirements for retrieval of charts and responding to queries related to patient information, and maintaining organized and up-to-date files and record-keeping systems.

 

·         Responds to requests for patient information in a timely fashion by proving copies or originals of chart information to our internal and external customers using appropriate methods including but not limited to phone, fax, copying, mailing, etc.

·         Maintains up-to-date records by insuring that all chart information, including by not limited to paper records, films, tracings, videos, log books, etc. are filed and stored in a timely fashion according to required time frames.

·         Maintains appropriate documentation and regularly updates record-keeping systems to properly reflect the status of request for information.

·         Meets all requirements for processing paperwork and coordinating all procedures related to death disposition of expired patients.

·         Coordinated organ retrieval with body release to Medical Examiner and funeral homes.

·         Performs required procedures to insure that duplicate medical record number assignment is identified and corrected in time frames as specified in departmental policies and procedures.

·         Participates in department staff meetings, training sessions and hospital quality improvement activities as required.  Recommends alternative methods to improve department operations.

·         Performs auxiliary chart completion / chart coordinator functions by following-up all incomplete charts routed from the hospital care areas and determining required action.

·         Insures that all requirements for a complete medical record are met by obtaining required signatures, insuring proper completion of forms, etc.

·         Updates computer systems as required to properly reflect the status of incomplete records.  Performs deficiency analyses to track and trend patterns.  Communication with physicians.

·         Scanning Duties: Prepares documents for scanning.
